Thank you for being a part of the NAFCS family.  Below is the current benefits guide that gives you a snapshot of all the benefits that are available*.  Additionally, within this page you will find more information on all the benefits that include detailed benefit summaries, value adds and much more.

Medical Information

As part of being a Humana member you also have access to several benefits as a result of that.  These benefits include but are not limited to Humana Vitality and EAP.  The document below explores those member value adds.

2018 Summary of Benefits & Coverage

These documents are required to be accessible due to the Affordable Care Act.  In the Summary of Benefits & Coverage it goes more in depth on how the benefits pay and offers a few scenarios to help guide you on how the benefit works.


Did you know?
As part of your medical coverage, you are allowed 1 basic eye exam visit per member per year at an in network Ophthalmologist. Go to Find a doctor; to access updated lists.

Dental Information

CompDent (Closed Enrollment; you can only make changes on this plan if you are currently enrolled on this plan)

Health Savings Account (HSA) Information

A Health Savings Account is only available if you elect to have the High Deductible Health Plan (HDHP). The HSA gives you the ability to use pre-taxed deductions to save for healthcare costs including dental and vision. If you are enrolling in the HSA for the first time, in order to get the Corporation’s contribution or in order to contribute from your pay, you have to open an HSA at Mainsource Bank on Grant Line Rd.


Flexible Spending/Dependent Care Account (FSA/DCA) Information

We use Ameriflex as our Flexible Spending/Dependent Care Account administrator.

A FSA allows you to use pre-taxed dollars on healthcare costs including dental and vision.  With an FSA you elect an annual amount to be withheld from your pay.  That amount must be spent by the end of the grace period or that money is forfeited.  In most instances you will be able to use your FSA card supplied by Ameriflex.  However, if it is not able to be auto-substantiated you will have to supply receipts.

A limited FSA is for use if you have an HSA.  It can only be used for dental and vision expenses if you have an HSA.  If you are not contributing to an HSA but have a High Deductible Health Plan, you can have a full FSA and use it for medical, dental and vision.

A DCA allows you to use pre-taxed dollars on dependent day care costs.  With a DCA you elect an annual amount to be withheld from your pay.  That amount must be spent by the end of the grace period or that money is forfeited.  To get reimbursed on a DCA all receipts showing payment to a qualified childcare provider must be submitted.

Below please find the benefit summary on how the plan works.  Anthem is the carrier for this benefit.  To find in network providers go to>find doctors and select Blue View Vision product.

Colonial Life is our voluntary benefits provider.  Below is plan information and cost.

403(B) Information

As a NAFCS employee you have the opportunity to contribute to a 403(B).  We use Edward Jones Financial Advisers with One America Investment lines.  Edward Jones Advisers enroll you and/or make changes to your 403(B) program.
