Medical Information

Effective 1/1/2017, we will remain with Humana for medical coverage. We continue to offer four plans:  $500 individual/$1000 family deductible plan, Office copays $20 primary care/$40 specialist; Coverage 1st $1,500 single/$3,000 family, Office copays $30 primary care/$50 specialist and two high deductible plans (HDHP) at $2,650 single/$5,250 family and $5,000 single/$10,000 family.  Below are the benefit summaries for all the plans.

As part of being a Humana member you also have access to several benefits as a result of that.  These benefits include but are not limited to Humana Vitality and EAP.  The document below explores those member value adds.

Health Savings Account (HSA) Information

A Health Savings Account is only available if you elect to have the High Deductible Health Plan (HDHP). The HSA gives you the ability to use pre-taxed deductions to save for future healthcare costs. If you are enrolling in the HSA for the first time, in order to get the Corporation’s contribution or in order to contribute from your pay, you have to open an HSA at Mainsource Bank on Grant Line Rd.

For new HSA members to start a contribution please contact Deena Burks. YOU HAVE TO OPEN AN ACCOUNT THROUGH MAINSOURCE BANK TO RECEIVE CONTRIBUTIONS TO YOUR HSA.

Vision Information

Colonial Life is our voluntary benefits provider.  Below is plan information and cost.

403(B) Information

As a NAFCS employee you have the opportunity to contribute to a 403(B).  We use Edward Jones Financial Advisors with One America Investment lines.  Edward Jones Advisors will be doing one-on-one meetings to enroll you and/or make changes to your 403(B) program.  They will be in buildings starting in November.  See school Administrative Assistants for sign ups.  Any enrollments done in November will take effect on the first pay of December 2016.
